Under this framework, a generic manufacturer is not required to repeat the extensive safety and efficacy clinical trials conducted by the originator. What they must demonstrate, rigorously and to the FDA's satisfaction, is bioequivalence to the reference listed drug (RLD).

The RLD for oral ivermectin 3 mg tablets is Stromectol, originally marketed by Merck & Co. Understanding bioequivalence—what it means, how it is tested, and what its clinical implications are—is the central question this analysis addresses.

What Bioequivalence Actually Means

Two drug products are considered bioequivalent when their rate and extent of absorption—measured by pharmacokinetic parameters Cmax and AUC—are statistically equivalent. The FDA's standard criteria require that the 90% confidence interval for the ratio of the test product (generic) to the reference product (brand) falls entirely within 80–125% for both Cmax and AUC. This range is not arbitrary; it reflects the threshold within which clinical outcomes are considered statistically indistinguishable.

Does this mean a generic product's AUC might be 20% lower or higher than the brand? In theory, the widest permissible range allows this. In practice, approved generics almost universally cluster far tighter—the mean difference typically falls within ±5% of the reference. The 80–125% window is a worst-case tolerance, not a typical outcome.

Ivermectin Bioequivalence Studies: The Evidence Base

Multiple generic manufacturers have conducted standard two-period crossover bioequivalence studies comparing their ivermectin 3 mg formulations to Stromectol under identical fasted and/or fed conditions. These studies enroll healthy adult volunteers, measure plasma ivermectin concentrations at defined intervals over approximately 96–120 hours (capturing the full half-life curve), and submit the resulting PK data to the FDA for statistical evaluation.

Published and FDA-reviewed data show that generic ivermectin 3 mg products meeting Orange Book AB-rating standards consistently demonstrate Cmax and AUC ratios within the 90–110% range relative to Stromectol. This is a clinically narrow band. The products are, in terms of systemic exposure, effectively interchangeable.

Formulation Differences: Do They Matter Clinically?

Brand and generic tablets may contain different excipients—binders, fillers, disintegrants, and lubricants differ between manufacturers. These inactive ingredients affect dissolution behavior in vitro, which is why bioequivalence testing uses in vivo human PK data rather than relying solely on dissolution testing. An FDA-approved generic with an AB rating has cleared both in vitro dissolution criteria and the more demanding in vivo PK equivalence standard. The excipient differences are pharmacologically inconsequential in the bioequivalent product.

There is one practical note: patients with documented hypersensitivity to specific excipients (e.g., certain dyes, lactose, corn starch) should consult their prescriber about appropriate product selection. This is a patient-specific pharmaceutical consideration, not a systemic concern about generic bioequivalence.

AB Rating and Clinical Substitutability

The FDA's Orange Book assigns "therapeutic equivalence" codes to approved generic products. An AB rating means the generic is therapeutically equivalent to the RLD and may be substituted in the absence of prescriber instruction otherwise. Generic ivermectin 3 mg tablets from multiple manufacturers carry AB ratings against Stromectol.

What this means for clinical practice: a pharmacist may, in most U.S. states, legally dispense a generic substitute unless the prescriber writes "dispense as written" (DAW). From a pharmacokinetic standpoint, the substitution carries no meaningful efficacy or safety difference. The economics are substantial. Generic ivermectin typically costs a fraction of brand-name Stromectol, which has direct implications for patient access—particularly in outpatient settings where insurance coverage is limited.

Quality Manufacturing and Regulatory Compliance

Bioequivalence approval is necessary but not sufficient to assure product quality.
